I don't know how the language patches work but you might want to install italian version of mafia. and after that run the patch as admin in Windows XP compatibility mode.

Sure I have the italian version of the game. I don't remember about running the patch as admin. I will try! |

Thanks risan,
the patch worked. Maybe because it was in a usb key and not in the desktop.
Anyway I used it s admin and Windows Xp SP2 compatibility mode. |

It might be some compatibility problem or you don't have Italian version of mafia installed. Make sure you have the same version of mafia as the patch you want to install |

I have a "combo setup" which ask the user to choose between Russian and English version. I torrented this down so many years now, I can't remember when.
Also, when I downloaded a mod (Slovak mod or something like that) surprisingly that came with the Czech version of the game, basically it included the whole game folder, not just the mod itself. 
Then, I found a "Green Edition" on torrent, but it turned out that is only the original game with all menus and subtitles are Chinese! 
So, basically I have these versions:
- Russian (R.G. Mechanics)
- Original (English) - "non-CD" version
- Czech
- "Green Edition" - Chinese menus & subtitles + original speech |
