You haven't told us what version of Windows
you have. I can only help with Win 10

Sometimes reinstalling Mafia under compatibility
settings might help.

if you are installing from the 3 PC-CD-ROMs

When you have CD#1 loaded don't click
on install straight away from the launcher
but open the contents of the disk in file explorer
and right click on MafiaLauncher.exe and then
select properties go to the compatibility tab
and select Run this program in compatibility mode
for Windows XP SP 3 and tick the run this program
as administrator (this assumes you have administrator
rights) - then click on install - the rest of the disks should
follow suit (as administrator and compatibility mode you
have selected)

Tip: one thing I have learned is that from Win7 onward
any executable file is best to set yourself by right clicking
the file and under security and then the edit button to
go to your name (computer) and ticking the full control
boxes. And even after this, when you start any program
right click and run as administrator.

I can say that the italian version of Mafia never got a proper patch 1.3 from what it seems...
