If you install MWE to a directory other than your Mafia installation folder, you need to edit the MafiaPath setting in Editor.ini to point to your game installation.

GOLOD55's Real Driver Pro mod does if I'm not mistaken.

Aside from that you can use mse_car_setsiren command which you could bind to some key via ctrl_read(ex).
